Needs Assessment $90

Feel like you need some help but don’t know where to start. We’ve got you covered.

A Needs Assessment is a great introduction, we will meet you in your home, spend a hour talking and getting an idea of what needs are not being met. We will ask you questions and you will have lots of time to tell us what is important to you.

After the hour we will leave, and spend time doing some research to come up with lots of options for you and a care plan proposal.

We will then come back to visit you and share with you our ideas and options on how to best help you with your specific needs. We will have another hour to chat and agree on a plan going forward.

This could be a personal monthly care package, maybe the help that you need requires support from others which we can arrange. Either way we will ensure you feel cared for.

Needs Assessment includes: 1hr initial visit, 1hr+ research, 1hr follow up visit

Needs Check

I will give you a list of human needs and you can check any that are not being fulfilled.

Is there something in particular that is an issue for you? Let me know and I can find some possible solutions.
